Can the Spreadsheet to Form widget retrieve data from two Sheets?

Is there a way to have different fields display based on which code is entered from the spreadsheet? Ideally I would upload a spreadsheet with 2 sheets, each with one column of unique codes. If a code is entered from Sheet 1, certain fields display and different fields display if a code is entered from Sheet 2.

Thanks for reaching out to Jotform Support. What you have in mind is possible in Jotform. You will be adding two spreadsheet to form widget. After adding them, you will also add the fields you want to show when the user entered the code in Spreadsheet 1 and do the same for spreadsheet 2. Once the fields are set, you will have to add a show/hide conditions. We can help you get this implemented on your form if you can share with us the link to your form.

I don't think I made myself clear. The form would not be viewable until the passcode is put in, and different fields would display depending on the passcode inputted. I already have Spreadsheet to Form added, and a 2nd wouldn't make sense - it's the first item on the form, and can only verify the codes from one sheet of codes. Thanks for getting back to us and for the additional information. Please note that when you configure your Spreadsheet to Form widget, you have to select the Sheet from which the widget should get the data. 

That being said. At the moment, the widget only can get the data from one sheet. So as a solution, you can add all the codes and data in one sheet and make sure that all codes are listed under the same "Access code column".
