Formulario repetidos

Hola buen día! Veo que me llegan varias veces el mismo formulario, al parecer le dan varios clic al boton de enviar.

Ya intente ponerle un captcha a los formulario pero sigue pasando, podrían decirme como evitar que se autocomplementen los formularios?

You can set your form to have the Unique Submissions enabled, and have the setting to Check cookies only. With this, it shall prevent users from accidentally making multiple submissions using the same browser.
