How to make questionaire with radio buttons?

Asked by Fireconsultingltd on December 09, 2016 at 08:06 AM

I have a list of Yes, No N/A questions, of which some are sub and require indenting. Is this possible?

Is there a dedicated Yes, No, N/A widget?

    Hi,

According to my understanding, you want to make a form that user needs to select one of Yes, No or N/A options.

You can use basically Radio Button to create a question with Yes, No, N/A options. After that, you can duplicate this question by changing the text of them.

Please have a look at the following images:

#1- Add Radio Button in Basic Fields to your form:

#2- Duplicate your Radio Button Question.

#3- You can make your questions Required. First select your form field and click Required at the top of the page.

Please have a look at my demo form: https://form.jotform.com/63433983887978

You can clone the demo form to your side. This is the guide about how to clone: How-to-Clone-an-Existing-Form-from-a-URL

If you want to change the style of the radio buttons, you can inject some custom CSS. If it is the case, please feel free to let us know. You can follow the guide: How-to-Inject-Custom-CSS-Codes

If it is not the issue, please feel free to let us know.

    Thank you.
