How to make questionaire with radio buttons?

    I have a list of Yes, No N/A questions, of which some are sub and require indenting. Is this possible?

    Is there a dedicated Yes, No, N/A widget?

    Hi,

    According to my understanding, you want to make a form that user needs to select one of Yes, No or N/A options.

    You can use basically Radio Button to create a question with Yes, No, N/A options. After that, you can duplicate this question by changing the text of them.
