All of a sudden, the embedded forms we use regularly are not appearing on our pages. This is the height of our busy season, so an explanation would be greatly appreciated.

When did it stop showing? I checked your form, and it is displaying fine, any message you got during the time it was not showing?

Hello, I believe it was working last night but it's not appearing on that page. We also aren't seeing our JOtform appear on this page: https://newmedia.tcu.edu/services/requests/
I had a student check on his phone, too, so we could see if it appeared on a separate device. Nothing showed. Could the embed codes have changed?

I have checked the forms in your account and I can see they are working, indeed the form embedded on the link you have provided works fine on a desktop device as well as on a mobile, here're some screenshots about it:
As far as I know the embed code has not changed from our end, what I noticed is that you're using the default embed code (JavaScript), could be possible for you to try re-embedding your form using the iFrame code?
The iFrame code is recommended to avoid conflicts between the form's code and the existing code in your website, here's the guide to get it: Getting-the-form-iFrame-code
