This is the third email I've sent regarding an issue in which salesforce integration has not been processing properly.
I've verified all fields in the past and it worked. I cannot now authenticate my account in order to log in and make edits.
Could someone please contact me in order to get this issue resolved as it's costing us productivity.

As indicated by my colleague there, please make sure that you are copying the password or the token carefully so that non-printable characters are not copied.
Also, ask SalesForce support if the security token is not longer valid, and if it is necessary to create a new one.
Also, once you make sure your credentials are correct, do the following:
1) Remove the integration.
2) Clear your forms cache: https://www.jotform.com/help/229-How-to-Clear-Your-Form-Cache
3) Integrate back again: https://www.jotform.com/help/271-Flexible-Data-Integration-With-Salesforce
