How to include PDF attachment of submission in Autoresponder?

    I would love it if the user entered in their email at the end and received a pdf/screenshot of the completed form with answers to their email inbox.  

    I would also like it if the pdf looked like the actual form (I have graphics embedded in headers that are critical.

    You can enable that option by going to Emails section and turning on PDF Attachment:

    With that enabled, your users will receive submission as PDF to their email.

    Regarding your images, I see they are presented in Headings. To include them in PDF please check this guide:
