How do I get the template name changed or deleted?

May we know what you are referring to? Are you referring to a template that you shared that is displaying this page: https://www.jotform.com/form-templates/? Or are you referring to a template you want to use/clone?

I'm checking your account and I see that forms are shared to you by the account username WOLFEDEN.
