Another one of my forms is not mobile responsive

I clicked mobile responsive, but it cause me to lose my formatting. And it still wasn't responsive. Please help me fix and PLEASE help me keep my formatting. Thanks!

I have tested your embedded form on my Samsung Galaxy and I was able to replicate mentioned issue, full name field is not responsive. However your standalone form is also not responsive. 

Please add Mobile Responsive widget to your form. That should resolve your issue.
