Jotform services my website submit form: blackpearlsealcoating.com
Currently, if a customer wants to contact us through our website submit form, the same problem continues to occur. Once all the fields in the submit form have been properly filled out and submitted, an error screen continues to pop up stating the following: "INCOMPLETE VALUES; There are incomplete fields in your submisssion. Please go back and fix the problem." Though there are no errors in the submission, the error screen continues to pop up every time. I would much appreciate some assistance on correcting this problem. Please advise. Thank you..

I have checked your web page and see that the form is embedded using Source code of the form. Please try re-embedding the form on your web page using default javascript embed method or iframe embed method which should be working fine.

On your MyForms page, click the form and select "Edit Form"
Then, on the form's builder page.
You can also access the Publish wizard of your form through the MyForms page:
