I have tested the form and I was able to submit it without any issues. There is no error as far as I could see it. If you are still experiencing the problem, I would suggest re-embedding the form but this time, get the iframe embed codes instead of the default.

I suspect you are experiencing a script conflict in the page causing you the problem.
