My customer could access to their form, with a password and user name

Hi, I would like that my customers login as user name and password to their forms, and also I could upload pdf and jpg, documents to the forms, after the submision.

I'm sorry, but we do not allow forms that would collect such details from users. We consider that as phishing which is against our Terms of Use. But file upload is possible in the form.
