Last night I was working perfectly and I even made some test responding the form and everything went smooth, but I tried to do the same this morning and when I tried to open the preview this happened, could you please help me to fix it cuz I don't know what happened, I didn't do anything besides trying to open the preview form.

This was caused by a form cache issue. I have cleared your form caches and now your form is displayed as it should be. To resolve such potential issues you might have with form caches please see how you can clear them yourself on this simple guide:
