the form on this page:
the data is being emailed to the right person, but i don't see any submissions recorded in the list of submissions that i can access on the My Forms page.
the last one shows 2015. there should be some from within the last 2-3 weeks.

the submissions started coming in on Jan 2, 2017.

I have checked on your submissions page as well as email history and found that the first submission for this year was made on Jan 2, as you already noticed.
If the emails were sent then the submissions must be displayed in the submissions page, I would suggest you to check the email history in your account and compare the number of emails that were sent related to a submission as well as the submissions in your form.
I have also submitted your form and I can see the emails and submissions were recorded, is the issue resolved on your end now?
Please let us know if this persists.

so the email where the form data was sent to somehow got on the Bounce list. i have just removed it from that list.
how can i get a list of the all the submissions that were bounced back?

You can get the complete log of the emails by following the guide given above, the blocked/bounced emails look like this:
Also, do note that you must enter the email address in the email wizard instead of typing the recipient's name, so it should be "email@example.com" instead of "First LastName".
