How can I set quiz answers on Brain Daze template

I followed the procedures and created the quiz. I used Brain Daz quiz as my
template and I don't know how to set the answers on the quiz. It is showing
3 details first name, last name and email.

To use the Quiz App, you need to have a quiz form with questions on it. The form should have radio buttons or checkbox on it to evaluate the answers. For example, the form (https://form.jotform.com/70241827679969). Then to turn it to quiz form, all you need is to navigate to: http://quizform.jotform.io/ and perform the instructions provided in this article: Turn your form into a Quiz or Exam!.

To set the answers, just select the correct answers on the form, and save quiz. Please see the animation below:

I did follow it but the questions were not visible while clicking on "set
answers"

Hi there,

I can't seem to replicate the problem you are having. In the GIF below, I can successfully set the answers to quiz questions.

 

Can you please give us more information about the problem such as the browser you are using?

If possible please also open the console of your browser by pressing F12 and opening the Console tab and take a screenshot of it.
