I tried adding the Smooth Signature widget to the form and it never loads or shows up.
Is there another step I am not aware of?

We are currently not having any issues in our widgets. I've tested the Smooth Signature widget on a blank form and a cloned form, it is working properly.
I cloned your form and I didn't see the Smooth Signature widget on it. Please try adding the widget one more time by dragging it to the work space.
If it is still the same, please log out from your JotForm account. Disable any browser add-ons or extensions and then log back in to your account. After that, try it one more time.
You can also try using another browser just to check if it will work or not. Lastly, you may need to clear your browser cache.
