I am trying to convert the IRS Form W-4 (2017) to a JotForm. I could build the form by hand and gather the same information but would like to keep the formatting the same or as close as possible. I do not want to embed this form for download, I want it to be a fillable and signable form.

This is the link to the PDF Form I want to conver: https://www.irs.gov/pub/irs-pdf/fw4.pdf

Any help with doing this is greatly appreciated.

ou can import your PDF forms from the link below:

    http://www.jotform.com/adobe-formscentral-alternative/

From there click on "Import Your Forms" button at the center of page or scroll down and click on "Import Forms" button at the upper right corner. Then click "Upload" button on PDF only option.

Some PDF's cannot be fully imported and may not retain their formatting.  You can give it a try but the form may need to be recreated from scratch.  I searched our templates for a similar form, but was not able to find one.

Ok, I received an Import Failed error so I guess it cannot import it. I will try and see if there is another way or just build it from scratch.

Creating it from the scratch would be the best option, since that import PDF feature is meant for FormsCentral Fillable PDF forms.
