Configurable list responses not showing in email submission.

    I have a chart in my form with check boxes. Those boxes are being checked but not coming through in the email submission. See the Services Required section of my form please.

    It appears that the checkbox option doesn't have any names on it, so the selection isn't visible in the email. You will need to add names for the checkbox so their names will be displayed in the email if the box is checked.

    When I check the boxes, here's the result in the email.
