I am needing help capturing information on my form

I am creating a form to send to group that are attending our campus for summer conferencing.  I would like to capture the following information: 

Date       Meal                       # of people

              Breakfast

              Lunch

              Dinner

Typically there would be multiple dates with multiple meals and various amounts of people.  What would be the best way to capture this info?
