Customers cannot submit form due to an Exception Error

    hello, 

    we have had our form working propoerly for serveral weeks. 

    We made a few changes today to some wording and such and now my customers are getting the below error message when they attempt to submit the form.

    issue seems to have resolved itself for the time being, can you advise why this may have happened so we can be prepared for next time if it happens again

    I did a test submission to your website and I was not able to replicate the issue. I was able to submit the form successfully. See the image below:
