Hello,
We're getting ready to subscribe/upgrade our account with you. Before hand, please explain:
Rather than embed the form code I've built within my free account: https://form.jotform.com/70315333821144
I'd like to simply use the live form at that location (above). However, I'd like for it to not show "form.jotfom.com..." when a visitor views it. Is it possible to customize that URL, without having to build and host a brand new page?

You can customize the URL of the form. Please refer to this guide: How-to-create-a-Custom-URL-of-a-form.
You can also use your own domain names. Here's a guide: How-to-use-Custom-Domains-with-Jotform.

But if I want to customize the URL, does it have to start with https://form.jotform.com...
?
When we move to a paid plan, I'd like to not show jotform anywhere.

The custom URL option will still use the JotForm domain. The URL contains the domain, username and form title. Here's an example:
https://form.jotform.com/Jan/registration-form
Please take note that this option is available for Free and Paid plans. If you don't want to use our domain, please use a custom domain. Here's a guide: How-to-use-Custom-Domains-with-Jotform. Here's an example:
http://forms.mycompanydomain.com/form/1234567890
