How to mix the inputs table and the gift registry widget?

I would like to mix the inputs table and the gift registry widget. There is an online date reservation form with a lot of dates which can be arranged on table. Do you have any idea to do this?

I got these dates and times. Everyone who fills the form have to choose one, and if you choose one no other can choose the same. It is too much data to write it line to line so it would be better to take it in a tablesheet. I couldn't find a solution.
