You can change a custom domain name of your form. Please refer to this guide on How to use Custom Domains with Jotform.

When you embed your form on your website, it will be searched and available to be indexed by Google. If you want to hide the form from search engines, you can add noindex and nofollow robots meta tag to the webpage with embedded form.

<meta name="robots" content="noindex, nofollow">

You can also add the parameter re="nofollow" to the form link. For example: 

<a target="_blank" href="http://form.jotformpro.com/form/1234567890" rel="nofollow"  rel="nofollow">Form Link</a>
