Am I able to upload a consent form that I only need a signature on? Like how Docusign does? Or do I have to make each form all over again?

Hi Sheila,
Thanks for reaching out to us for help. It’s easy to create a Signable Document. Let me show you how:
- Go to your My Documents page and click on Create Sign Document.
- Now, you can upload your own document, or you can use the Demo Document to test it out.
- Then, you can add more documents, or you can just continue by clicking on the Create Signable Document button.
- That will redirect you to the Sign Builder page.
After creating your document, you can let the system detect fields automatically by clicking on the Detect Fields button, or you can manually match your fields with elements by dismissing the pop-up message:
Once your Jotform Sign document is created, it can be sent to different people, without needing to re-create the document again. Let me show you how to publish your sign document:
- In Sign Builder, in the green navigation bar at the top of the page, click on Send.
- In the Send To Sign section, you can easily send your document so people can sign it.
- Just enter a name and email address in the related fields, and click on Send To Sign.
You can also see the Sent Signable Documents as well at the bottom of the Send section:
You can also Send Reminder Emails, Set an Expiration Date, Add a CC Recipient, and Allow Signer Delegation under the Options tab:
