Why are my forms disappearing and reappearing, only to disappear again?

I have checked the pages you have linked to, and I was able to reproduce the issue you are describing. When reloading the page, sometimes the script that loads our form doesn't seem to be present in your webpage:

This seems to be an issue from your site itself where at times it doesn't include the script that loads our form. In case that this behavior is being caused by some other scripts that are used in your website, I would recommend that you please re-embed your forms with our iFrame embed method instead:
