My enquiry forms were administered by Cosmetic Digital and I would like to pay the subscription to continue with Jot Forms.

    I want to get this sorted ASAP, as I am losing customers via the website enquiry forms.

    Hello Emma,

    I did check the form which is embedded in your website and found your account username is "cotswoldfab". The profile email address of this account is cotswoldfabclinic@gmail.com 

    Do you have access to this email address? If yes, you can reset your account password by clicking on the "Forgot Password" link which is in "Sign In" page. Please check the screenshot below:
