For some reason, while the phone number field correctly displays in the form builder, it's splitting the area code and ph number onto two lines in live form. It didn't used to do this - so this is a new-ish development. Help meeeeee!

The space the form is being embedded to is small enough to trigger mobile responsiveness. This is changing the form to mobile format.
You can adjust the width of the area your form is embedded to:
Adjusting this to something like style="width: 590px;
Or you may need to replace your current embed code with you forms iframe code:
If that does not work, let us know and we can help you adjust the width of the frame.
