I haveintegrated mailchim form but i dont know how to work

I want to create a form that when user enters their details go straight into a mailchimp list.  I have created the list in mailchimp, created the ignup form, matched the fields and set up the api.  However, on my form I cannot see any entry fields https://form.jotformpro.com/70993420592966

When you say "However, on my form I cannot see any entry fields",do you mean that the submissions are not found in your submissions page? https://www.jotform.com/help/269-How-to-View-Form-Submissions 

Did you check in your MailChimp list if the submissions made after the integration were sent?
