'Oops! Something went wrong' error while editing form

We keep getting an error that says "Oops! Something went wrong and We couldn't initialize a part of your form correctly." when we try to edit our form, what is the issue? we need to edit the form aSAP!

I have checked your JotForm and see that the error is being displayed while editing the form. Did you make any changes to the form recently? Could you try reverting the form to the last working version from the form revision history? Please check the following guide that can help you with form revision history.

There has been an issue affected some forms but it was fixed. Please check and let us know if the issue persists. 

No we did not make any changes it was working fine and stoped working this weekend.
