Jotform Template

    Hi Guys!

    I would like to re-create the following form in Jotform:
    http://atfgyms.com/checkin/

    I would like to be able to match the exact style and functionality.
    Please help!

    I checked the site you have provided (http://atfgyms.com/checkin/) and I can see the style and functionality you are referring to.

    What I think of the possible way to accomplish this is to add page breaks on the form. Just like in the form templates below:

    However, I'm afraid to say the Sliding functionality can't be accomplished in the JotForm Builder alone. So, to accomplish your requirements, you need to download the full source code of your form and add a code to make the form slide as what you are seeing from the link you have provided.

    You may take look at this link: http://jformslider.com/home#demo where I have found the same functionality.

    I think that functionality would work great as well!  Can you help me design a duplicate form of the one I'm trying to create with the same branding/design?

    We can use the functionality of the form you referenced above.

    You can try cloning the form (https://form.jotformpro.com/form/41321165563953) and start editing the form according to your preferences. Here's a guide on How to Clone an Existing Form from a URL.

    Hello!

     

    i tried cloning it, and I can do that.  But I really need help, because it doesn't look anything like the form I am cloning.

    the font style, text box size, colors and spacing need to be the same as the original, and I'm not sure how to accomplish this.

    I noticed that the styles on the form (https://form.jotformpro.com/form/41321165563953) are not the same as the original template when it was cloned on my end. However, I have tried to clone this form (http://form.jotformpro.com/form/41945511112950) and I found a good result.

    If this is not what you want to accomplish on your form, you can try adding page breaks and customizing it using CSS. Here's a guide on How to Inject Custom CSS Codes.
