This Form is Disabled.

    Hello,

    I have recently completed a form. However, when I put the link on my website to redirect people to the form, it seems to have trouble opening. It continually says that the site cannot be reached. I have tried to redirect to the form in multiple links and none of them seem to be working. I don't believe its an internet issue either, because the link isn't working from multiple locations. I'm wondering if it's an issue with the form itself? Not exactly sure. Please get back to me with any helpful information.

    I have included the website URL. When you look on this page, the form I am trying to access is supposed to be linked to the "Step One" box where it says "Fill in the online application form here"

    The link to your form from your site (http://www.breathebayarea.org/asthmacampsuperstuff-957106.html )  actually works. I was able to be redirected to your form. However, I assume that the issue you are having is this Form not Found Error:

    This means that your account was auto-suspended by our Anti-Phishing Tool that made your forms disabled.

    I checked manually your forms and I noticed that the form (http://www.jotform.us/form/71019316957157) has fields that ask credit card details from the users. Please note that doing such an act violates our Terms of Use

    If you would like to collect payments from the users, please use the Payment Processing Tool on your form. Here's a guide: Order Form Basics

    I have now removed the guilty fields on your form and re-activated your account. Please refrain from doing this again so that your account will not be suspended.
