I have checked on our logs and found that you received the email about the failed attempt to import your PDF form, please do note that this will require to create the form in our Form Builder.
Here are some chapters from our guides that will help you to start building your form:
https://www.jotform.com/help/chapter-1-Getting+Started+with+JotForm
https://www.jotform.com/help/chapter-2-Create+Forms
The form still appears on your forms list, but it cannot be modified and it will simply load blank.
If you have any question, please let us know.

It the form you were trying to import was from Adoble FormsCentral then it should work, otherwise that may be the reason why it did not import, here's a guide about it: https://www.jotform.com/help/340-How-to-Import-Adobe-FormsCentral-PDF-Forms-to-JotForm
I would suggest you to instead create the form through our Form Builder and then you could export it as PDF fillable form: https://www.jotform.com/help/212-How-to-Create-a-PDF-Form
Here's also a guide that may help you while creating your PDF form: https://www.jotform.com/help/312-JotForm-Fillable-PDF-Creator-With-All-Points
Hope this helps.
