How can I add 2 tabs on facebook?

I added one tab for the job application at our facebook page. Wanted to add another tab for the customer survey. Can we do that?

First tab that I added on facebook, that appears on desktop facebook. But does not appear on iphone Facebook App or iphone chrome browser.

With regards to viewing the form (app) on your Facebook page using your mobile phone, I afraid it is still not possible with the Facebook App or with a mobile web browser.
