One response comes to me via email as usual.
A second identical response comes to me in email with the subject: "New Submission".
Both are identical.
How do I stop one of them?

I have checked this form http://www.jotform.us/form/71143855719159 and found you have two identical email notifications:
I also noticed you have an email auto-responder with empty recipient field, please either select a recipient or delete the emails.
