Forms not sending out e-mail notifications

There are three forms on this website that have been functioning for several years, but not notifications have stopped going out. I can see that the inquiries are coming in, but no one getting the notifications.

In addition, the "place an order" form has disappeared from the list of "My Forms"

I could not locate any jotform in your site. Please share the exact URL of the page within your site where your form is embedded.

Please check if the notifications have landed in your spam folder, and mark them as NOT SPAM if you find them there. Also, check if your recipients email are in our bounce list: https://www.jotform.com/help/262-How-to-Remove-Your-Email-Address-from-Bounce-List
