I did check your form and found that you have added 3 notification email alters. That means every time your form is submitted, you will receive 3 submission emails. Please check the screenshot below:
I would suggest you to please delete 2 of the email alerts and that should solve your problem. The following guide should help you how to delete email alerts: https://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder
