Auto-responder doesn't seem to be working. Or maybe I have something set up incorrectly?

It’s called “TIME OFF REQUEST FORM”
I added the “email” field and have it defaulted so that an email address automatically populates (mine in this case). I am trying to set it up so that I can define who the auto-responder goes to based on information provided in the “company” selection.
For example, if the company “Florida Tool” is selected, I would like to set up conditional formatting so that it shows “email 1” with my email address. This way I can have the auto-responder come to me for the Florida Tool employees.
Then if someone selects a different company, i.e. Daniel Iron, I can have “email 2” show so that it sends the auto-responder email to the person in charge of the company Daniel Iron.
I am using this as a work around since the email notifications doesn’t offer conditional formatting. i.e. based on a response, you can define who the email goes to.

I have examined the autoresponder you have setup in the form that you kindly provided, and I noticed that the "Recipient Email" field is not set to a proper email field, which may explain why you're having issues with sending these emails:
