Form embedded in weebly website is not mobile responsive

the form is not responsive. please see the website on a mobile.

if i open the form from jotform itself it's responsve but now not with the code you sent me?

I have checked your website on iPhone 6, Safari. I have seen that checkboxes and radio buttons are not appearing well as seen below:

I have seen that you did not use the iFrame method in order to embed your form. Could you please try embedding the form via iFrame? Please find the iFrame codes of your form as you can see in the following guide: https://www.jotform.com/help/148-Getting-the-form-iFrame-code 

Then, please embed the iFrame codes to your Weebly website.
