How to remove the JotForm advertising?

I have just had a very annoyed customer call me to complain that the appearance of his website page (Contact us) has been spoiled by the appearance of a banner inviting his customers to create their own JotForm.  He's a Plumber, so there's no chance his customers will be interested in creating forms.

How do I remove this?

To remove the JotForm branding from your forms you will need to upgrade your account, once upgraded you will find the option under your account settings:
