Make subtotal for each item then grand total

I need to make a form looks like this: https://app.etapestry.com/onlineforms/IowaAgricultureLiteracyFounda/AgToday.html.

I tried form calculation but it didn't work very well. I want to show one item in one line and $ sign in front of the subtotal. Will you help me create this kind of form?

You can achieve that with using any of the payment fields under Payments tab. You just have to create your products, add quantities and enable total calculation.
