Form to be linked to WooCommerce

Hi, I have a situation: I'm using woo commerce product variations and attributes and so on  on my WordPress website. 

Problems that I'm facing are as follow:

I'm trying to sell business cards: when ordering, customers should have an option to choose more than 3 variations like 

1/ 1 side = $???

2/ 2 sides = $ ???

3/ Rounded corners bus cards

4/ laminated

Problems:

Customers cannot select more than 1 option the other option will deselect, is there anyways with your forms to have these functionalities and be linked to the shopping carts with woo commerce integrated?

Please let me know as quick as possible,

We do not have integration with the WooCommerce platform. I would like to suggest to get in touch with a wordpress/woocommerce developer to find out if some kind of integration is possible.

If you create a JotForm order form, you will have the ability to select multiple products.
