    I checked your web page, but I didn't find the form you're referring to.

    If you're still having an issue in regards to submitting the embedded form, then please re-embed it using the iFrame method. This will prevent script conflicts between the form and the web page. Here's a guide: https://www.jotform.com/help/148-Getting-the-form-iFrame-code.
