Email Notification missing form field

My form is missing a field in the email notification.

Is there a way to fix this?

i did not solve this issue yet- see attached email notification

Clinic Location field is hidden on the form and it is empty. By default empty field will not be shown inside of email Notification. You have set 'Atlanta' inside of the placeholder, but that is hint example. 

If you want that 'Atlanta' is shown inside of email Notification you should set it as Default Value.
