New form fields and responses not updating correctly

    I just made changes to this form - https://form.jotform.us/52695584375167

    When I sent myself a test, the current form is filled out and retrievable in the 'submissions' section through the jotform site with all the new questions and responses, but what is being sent to me in the email notification from jotform is the old form- no updates or new sections on it.

    Im not sure what the problem is. How did you test it? If you want to test your form you should go ahead and submit the form. It should send you the submitted data.

    I cloned your form and tested it. It seems to be working correctly. I choose South Texas-Houston/San Antonio and received two emails. One from the conditions. And one from the notifications.

    Please be more specific about what is not working. I would be happy to help you.

    The form is not sending me the correct info. Here is a pdf of what I
    received after the form had been changed, but it still has many of the old
    questions in it. The most obvious is the last field which still has the old
    agreement in it. When I retrieve the submitted form directly from jotform
    under the 'Submissions' button, it is the correct questions and answers.
    I'll have to contact our South Texas volunteers and have them forward the
    form you submitted to them to see what that one came through with.

    Sorry for the inconvenience.

    I understand that when checking the submissions on the Submission Page, the questions and responses are updated. But the submissions you received on email notifications, the questions are not updated. If this is the case, can you please try removing first the Email Notification and add it back on your form to see if this help to fix the issue?
