Here is the URL for my survey. When it comes into the receiver email it is fine and all the questions are easily read and the answers are there. But when the email is forwarded to my email the questions show up in a single line down the left side of page.
Also I cannot get the email to come to me from the autoresponder as the person filing out the form. Please advise.
Also, I am seriously having trouble getting the calculation of the satisfaction ratings to work. Thank you.

Our mail logs shows that notifications are being sent, have you checked the spam folder? If you find the notifications there, make sure to mark them as NOT SPAM. Also, whitelist our domain names and sending IP addresses within your email service provider: https://www.jotform.com/help/145-Whitelisting-JotMails-IP-Addresses
On regards of the autoresponder, can we do a real submission in your form to test it?

Hi BDAVID,I found out that Jotform had my email on their blocked list. Don't know why. So when I sent a survey today it came through. However, it is not coming in as the form. It is still coming in as a single line of text down the left side of page. Has anyone been working on this? Can you tell me what is happening? Is it a Yahoo issue? Please help. Thanks.
