My form will not submit

    I removed the captcha from the form because it was stuck in load mode.

    I tried submitting without the recapthace and the submit has been hanging in wait for over 5 minutes.

     

    Please advise.

    I was able to replicate the issue when I did a test submission on your web page. Here's a screenshot:

    It is possible that there are script conflicts between the form and the web page. Please try to re-embed the form using the iFrame method. Here's a guide: https://www.jotform.com/help/148-Getting-the-form-iFrame-code.
