Embedded form is not working properly

For one reason my embded code s working on some laptops mainly mac and not on others mainly windows

This is likely a script conflict in the page. It is a conflict when the form won't respond when you click on it or if the field validation won't work in the form.

Please try re-embedding the form using our iframe method: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code. The iframe will not interfere with your website script and won't cause a conflict in the page. Let us know if the issue persists. 

now i dont see the form at all

The embedded form is showing on my end. I tested it with Google Chrome and Mozilla Firefox, here's a screenshot:

Also, please provide which browser are you using when you encounter this problem. We need the operating system and the browser version as well.
