I need help separating submission choices made by users

I have designed a subscriber form to ask members if they want to either subscribe or opt out of a newsletter.

I'm looking to have emails sent back to me separated by the choice that the user makes. If they click subscribe, I was hoping the email capture will go into a Subscriber drive doc. If they click opt out, having that go into its own drive file so I can narrow the list of my contacts wishing to receive the newsletter. At the very least, having the jotform reply titles reflect what choices the user makes. Opt out or subscribe in the subject line of the form. Is this possible?

    thanks!!

You can create two notifications: https://www.jotform.com/help/25-Setting-Up-Email-Notifications

Simply put the words (Opt out, and Subscribe) in the subject lines of each notification:

Finally create conditions to trigger those notification based on the selection: https://www.jotform.com/help/167-How-to-Send-Email-Based-on-User-s-Answer
