Insightly CRM: Integrate Jotform when customer contacts us

Dear Jotform Team,

I would like to create a jotform based on what customers enters on the website and this is eventually integrated to Insighly.

So basically Customer goes on my website for Enquiry with pre defined set of questions. Once filled in, it creates a Jot form which them connected to Insighly to create an Opportunity there
