How to use input field and dropdowns on the same row letting the user add multiple rows

I have 2 requests i want to allow users to firstly add there name to the report, secondly on the dropdown form i want the trainer to add the learners name in the row but cant do this either , help please 

I assume that you need an input field that user can add his/her name and additional dropdown fields to select trainees. 

You may use Configurable List widget to achieve that.
