We have a Jotform that we use for our employees to recertify, is there a way I can get this to create a cert card based on the results that I can then in turn print.

    Factors to consider:

    Different logos based on company

Needs to be able to be printed on card printer.  (Magicard)  Cards about the size of a drivers license.

Expiration date on the card.   (Form does ask for date taking test.  Expiration date is always 3 years from when completed test.)

Do you have any ideas about how this could be accomplished.

Currently we use an Excel sheet. and crop the pictures manually.

I'm thinking of creating the image to be printed on the card in the email notification (content) that you will receive every time there is a successful submission. — How-to-Edit-the-Email-Template-for-Notifications-and-Autoresponders

With regards to changing the logo, you can create multiple email notifications with different logos set on the company logo and is send conditionally based on the user's answer — How-to-Send-Email-Based-on-User-s-Answer

As for producing an expiration date, this can also be computed using conditions. You will have 2 date fields on the form, one is set to show the current date and the other will show the computed current date + 3 years which is 365.25 days * 3. Both date field can be hidden in the form.

The only trouble I'm seeing with this kind of approach is fitting the card image into the card itself since the tools to edit the card image is limited in the email notification.
